This document outlines the scope, limitations, and proper use of your chaplain credential, including important legal protections for both you and the organization. Completion of this disclaimer is mandatory to ensure compliance with our policies and applicable regulations governing chaplaincy service.

Please carefully review, sign, and return the Legal Disclaimer Agreement at your earliest convenience. Failure to complete this requirement may result in a delay, suspension, or revocation of your chaplain badge and identification credentials. Return via email to info@ftgmedu.org.
